The True Cause of Heartburn / GERD


Heartburn is mainly caused by too much gastric pressure in the stomach. This gastric pressure causes stomachcontents to blow through the LES and up through the esophagus where it is then stopped by the UES and sent back down into the stomach. Increased Gastric Pressure can be caused by:

  1. Too many bad bacteria in the colon, this causes methane gas to travel upwards putting pressure on the stomach (SIBO).
  2. H. Pylori in the stomach producing gas causing stomach pressure to increase.
  3. Ingestion of gas causing food and drinks (beans, carbonated water, fermenting vegetable matter, rice, excess
    lactose).
  4. Ingestion of GMO’s
  5. Tight fitting clothing (belts mainly) putting pressure on the intestinal tract which forces the stomach up
    and increases gastric pressure.
  6. Incorrect posture which leads to more pressure being put on the stomach by the spine.
  7. Weakened LES due to increased pressure causes more stomach contents to be force up through the esophagus.
  8. Lack of digestive enzymes which cause poor digestion of food which can lead to gas buildup.
  9. Candida Overgrowth causing fermentation and gas production.
  10. Constipation that leads to an increase of toxins, gas and bacteria in the colon that slows gastric emptying.
  11. Lack of Chloride in the diet which can lead to decrease in stomach acid.
  12. Poorly Functional Gallbladder, Liver or Pancreas which can lead to either digestive enzyme or bile problems.

Gerd

 

Gastroesophageal Reflux Disease is a condition in which mucosal damage is done to the esophagus, from chronic stomach acid and pepsin being trapped between the stomach and the upper esophageal sphincter.  This “trapping” gives the patient the characteristic feeling of heartburn. Having GERD can cause injury to the esophagus, including esophagitis, strictures, or even cancer from chronic inflammation if left untreated.

 

The most common symptoms of GERD are heartburn and dysphagia (trouble swallowing). A person with GERD can also display symptoms like regurgitating acid and food, have a sore throat, chest pain, increased salivation and nausea. If GERD is left untreated it can cause a reversible condition called Barett’s esophagus where the epithelial lining of the esophagus turns into squamous or intestinal mucus epithelium and can cause a greater risk of  esophagus cancer.

 

Diagnosis of GERD is usually made by acknowledgement of symptoms by a doctor. But an endoscopy can be done to get an idea of how damaged the esophagus is and the general function of the LES and stomach. During an endocopy a flexible wire with a camera at the end is swallowed and takes pictures down the esophagus and into the stomach. One of the best tests used in the diagnosing of GERD is an esophageal PH monitoring
test. This test is sometimes performed in the hospital for at least 24 hours and during this test a flexible catheter with a PH monitor on the end is placed though the nose down into the esophagus. It can be uncomfortable to have tube down your nose and into your throat but it can be useful in the diagnosis if
you have GERD or not.

 

Conventional Medicine’s Failed Treatment’s for GERD / The
Average GERD “Cycle”



GERD Medicine Treatment

 

Conventional medicine tries to treat GERD with acid reducing medications, and if all else’s fails surgery. Acid reducing medications like H2 antagonists or the dreaded Proton Pump Inhibitors work by block the stomach’s natural function to produce stomach acid. In theory this sounds like a great idea for someone who has GERD because if you limit the stomach acid that can get into the esophagus from the stomach, a person with GERD will feel much better.

 

H2 Antagonist
– Examples: Pepcid AC, Zantac – H2 antagonists are the safest of the stomach acid reducing drugs they only last for about six hours and work by lowering
histamine levels in the stomach which lowers stomach acid production.

 

Proton Pump Inhibitors
– Examples: Prilosec, Prevacid – PPI’s are safe drugs generally as long as a patient takes the drug not longer than one month. These drugs inhibit the
hydrogen/potassium ATPase enzyme system in the gastric cells that secrete stomach acid. These drugs were invented because they last longer that H2 antagonists and usually block acid for 1-2 days. Patient’s who take PPI’s long term start to have a whole host of problems like B12 deficiency, magnesium deficiency, food allergies, SIBO, and osteoporosis.

 

The problem with acid reducing medications is once you get a patient with GERD to take them long term, it doesn’t correct any of the above problems that were the cause of their GERD in the first place. By doing this all you are doing is “masking” the symptoms. This also leads eventually to the average person losing the intrinsic factor of the stomach (becoming deficient in B12), it weakens the LES over time, develop a magnesium deficiency if they take a PPI, developing food allergies from improper food digestion, and even developing SIBO from the lack of stomach acid killing excess bacteria from their food. This makes a whole host of problems that the patient didn’t originally have, and since the cause of their GERD isn’t cured to begin with, now the patient is stuck on the PPI for the rest of
their life.


This “masking” of the symptoms also explains the worsening GERD one has after discontinuing the PPI. The body tries to up regulate stomach acid to take care of problems that started because of PPI medication being taken for so long (SIBO, bad protein digestion in the stomach, weak LES) and this causes worse GERD then they had previously because abdominal pressure is so strong in junction with the excess acid production.The average person goes back on their PPI and the cycle continues until the doctor recommends a useless surgery Nissen Fundoplication.

 

Nissen Fundoplication

 

A Nissen Fundoplication is a generally useless and harmful surgery for the average GERD suffer. When a person has the nissen procedure done they have the top part of the stomach (gastric fundus) stapled around the LES or the lower part of the esophagus sothat it is strengthened and reflux in the esophagus is stopped. Like conventional medicines GERD treatment, this seems like a great idea in theory. Studies show that 90% of patients are cured from their GERD which is great, right?

 

But the problem with this is that most of them are now left with a host of problems like “gas” bloat syndrome, dysphagia, and gastric dumping syndrome. A fundoplication can also come undone over time. This happens on average in about 10% of the patients that have had the surgery. “Gas” Bloat Syndrome is a syndrome where the stomach can’t properly expel gas through burping and even in some extreme cases a person can even burp or vomit at all which can cause extreme pain. “Gas” Bloat Syndrome has about a 41% occurrence rate in people who have had a Nissen done and though it can go away in about 2 months; occasionally for some people they have it until the surgery is either redone or reversed. This surgery should be avoided at all costs!

 

Treatment Protocol 1: SIBO

 

SIBO is a medical condition where a person has too many bad bacteria in their small intestine. This is usually caused by both a poor American diet and long term use of a PPI drug (Proton Pump Inhibitor). Long term use of PPI’s causes bacteria that would normally be eliminated by stomach acid, to survive and flourish in the small intestine. Also the American diet of fermenting carbohydrates causes the bacteria to thrive and produce lots of excess gas. The lack of stomach acid also causes food proteins to be partially undigested which causes allergies to develop and this causes even more gas to build up.

 

The bacteria also make protective biofilms (one of the most common examples of a biofilm is the “film” found on your teeth when you don’t brush for awhile) which make eradication with antibiotics very difficult.  The biofilm protect the bacteria from antibiotic treatment, bactericides, and probiotics. To destroy the opportunistic bacteria you have to also destroy the biofilm that it is hiding behind. This can be obtained by either breaking down the biofilm itself using systemic enzymes or by chelating the Iron out of the biofilm to break it up by using either Calcium Disodium EDTA or NAC.
 

The main symptoms of SIBO are indigestion, lots of gas, and horrible smelling deification, burps, and flatulence. Most people with IBS are walking around with SIBO as their main cause of  their problems. Other symptoms of SIBO include abdominal pains, constipation and/or diarrhea, food allergies, and low grade fever. One of the biggest correlations between a condition and a disease are Rosacea and SIBO. In one major study 96% of Rosacea patients were in remission or cured for at least 9 months after taking Rifaxmin to treat their SIBO.

 

A hydrogen breath test is used to diagnose SIBO. It is a non-invasive fasting test done in which a doctor has you breathe into a machine that monitors the excess hydrogen or methane that is released by the extra opportunistic bacteria in your small intestine. You are either given glucose, dextrose, or lactulose and results are collected at twenty minute intervals for at least 3-5 hours. If you produce at least 20 ppm of hydrogen or methane then you are positive for SIBO (but even a result of 12 ppm should be treated at the very minimum). Lactulose is the best to use because it isn’t absorbed by the intestinal tract itself. Glucose is easily absorbed by both the bacteria and the intestines. This means it can give a false reading because at least 17 ft of the small intestine will not be tested because of the quick glucose absorption.

 

Are Opportunistic Bacteria The Main Cause of Leaky Gut?

 

The excess bacteria in the small intestine can wreck havoc with your health and lead you digestion into a vicious cycle. The more opportunistic bacteria in your small intestine, the more food they consume which can lead to nutrient deficiency of B 12 and Iron, which then leads to the patient developing anemia. The bacteria also consume more nutrients that are now unabsorbed by the gut which leads to both more bacteria and gas production. This leads to abdominal bloating, pain, and flatulence.


The bacteria then start to decrease fat absorption which leads to stool problems with color / fat content and this also leads to deficiencies in vitamins A and D. The intestinal lining further degrades and eventually can’t digest larger food particles which cause food allergies and sensitivities (gluten first, then usually followed by fructose problems, then lactose digestion problems).

 

Bacteria then enter the bloodstream from the loss of integrity in the intestinal wall and start an immune reaction that causes fatigue, systemic pain, and elevated liver enzymes. Finally, the bacteria start to excrete acids that cause neurological and cognitive problems like brain fog and memory problems. The cycle then continues as the body’s Immune system tries to kill of the opportunistic bacteria which at the same time poison the body with acids and toxins. The vicious cycle then repeats itself in the patient, who becomes sick for a very long time.

H. Pylori are opportunistic bacteria that usually can only survive and multiply in a stomach that has low stomach acid. If a person has a normal stomach acid level, H. Pylori is usually kept at bay or are eliminated by the acid. H. Pylori are the cause of most ulcers and are the main cause of stomach cancer. Signs of an infection are ulcers, gastritis, or unrelenting heartburn (because the stomach increases gastrin levels and acid in an attempt to rid the stomach of Pylori, Pylori also causes excess gas in the stomach which expels the acid). The best diagnostic test to determine if you have a Pylori infection is a stool sample or at the very least a breath test. Blood tests are poorly at determining if you have a current infection or an infection in the past.

Treatment Protocol 10 – Constipation

 

Constipation causes multiple problems in the body and can manifest itself in different ways. Constipatio can cause toxins to build up in the body from a lack of transit time in the body. The body tends to reabsorb toxins from slow moving waste in the colon. Slow moving waste also ferments longer and causes a greater growth of good and bad bacteria, as well as producing more gas which can cause heartburn by causing slow gastric emptying and pressure and bloating in the abdomen.

 

The main causes of constipation are lack of magnesium, dehydration, lack of vegetable and fruit fiber, prescription drugs that cause constipation (opiates are a prime example), hypothyroid and adrenal fatigue, and lack of exercise.

It is important that you have the right amount of salt in your diet and that it is also the right type of salt so that your body can properly make hydrochloric acid. Gastric acid is made up of hydrochloric acid, sodium chloride and potassium chloride in the stomach.  I would take 1/8 tsp of a good quality sea salt, Real Salt, or pink salt in a glass of non chlorinated warm water upon waking to help adrenal function and to start digestion. If you have low stomach acid I would suggest drinking the salt water formula in small cup of warm non chlorinated water twenty minutes before every meal.

 

Treatment Protocol 12 - Proper Liver, Gallbladder and
Pancreas Function

 

If you have a health condition with any of these mentioned organs it can adversely affect your digestive health causing GERD, diarrhea, constipation, gastritis, SIBO, or IBS. Occasionally, the health of these organs can be determined by the color and quality of one's stool.

 

Liver - Poor liver function is usually hard to discover early because the liver is one of the only organs that
can regenerate, so in other words it can take a beating and keep on ticking. Jaundice or yellowing of the skin and eyes is usually the only physical sign of poor liver function. But when it comes to digestion signs of poor liver function include, unrelenting GERD in combination with a very light colored stool. These symptoms together could be a strong indicator for liver disease.

 

Gallbladder - Some of the signs of poor gallbladder function are abdominal pain after eating fatty meals, a patients's stool can range from being pale to yellow and usually appears greasy, and the odor is very strong from both the lack of bile and undigested fat remaining in the stool.

 

Pancreas - The signs of a poor pancreas function as far as digestion are the same as the gallbladder indicators of disease. Diarrhea can also occur with foods high in fat content. A symptom of poor pancreas function outside of the digestive tract is insulin resistance and diabetes.

Treatment Protocol 14 – Develop Proper Defecation Posture

 

Even though this is technically an intestinal recommendation, proper defecation posture is important to lower inter-abdominal pressure. So what is proper defection posture you might ask? Well if you guessed that it is sitting straight up on the toilet when you defecate, then guess again. You can always ask any toddler or dog / cat, what is the proper posture for defecation. And even though they can’t tell you how
they use the bathroom, they used the bathroom the humans were designed to use the bathroom, squatting.

 

The problems with sitting on the toilet instead of squatting are numerous. When you sit on the toilet it causes a narrow anorectal angle. This can be obstructive and can cause you to not completely empty your bowels when you use the bathroom. This causes some stool to be pushed back up into the colon. It has been theorized that this back up can eventually lead to appendicitis from constant irritation of toxins and opportunistic bacteria that were posed for elimination.

 

Also sitting on the potty to defecate causes you to repeatedly perform the Valsalva maneuver which puts stress on both the vagus nerve and the cardiovascular system as well. Finally, sitting on the toilet can increase diverticulosis and hemorrhoids from the straining that is required. The pressure build if the patient is constipated can be three times greater than if they had squatted to relieve themselves.

 

There are two benefits to sitting on the toilet are that there is more added privacy when you are sitting on the toilet, as well as less splash back from using the toilet that might get on your clothes. Sadly these two things are the only negatives for squatting on the toilet to relive oneself.

 

To squat on the toilet one has to stand with their knees and hips sharply bent and their butt should hover over the toilet opening. Some people who squat have seen complete relief from their constipation. The main reason that is theorized is it is easier to defecate because of the increase in the anorectal angle. You can purchase a squatty potty, which is a platform to stand on so one can successfully squat over the toilet very easily without having to balance.

 

Treatment Protocol 15 – Elimination of Parasites

 

Parasite infections are more common than one believes in this modern world that we live in. Parasites can cause digestive problems ranging from GERD to complete abdominal distress. A common myth is that more ingestion of parasites comes from eating meat, because of the horrors factory farming. This is a myth because animals even from factory farms are regularly tested for parasites, and also as long as the meat is properly cooked all parasites are killed from the heat of cooking. Most parasites in the human diet come from raw food, especially vegetables. According to the research most vegans actually have a greater parasite load then their meat eaters.

 

Most intestinal parasite diagnosis comes from a stool sample that can be taken by your doctor. A blood test can also be used in some cases to test for parasites.  

 

Piperine Theory / The Golden Bullet for Parasite Treatment?

 

Piperidine is used in both the treatment of parasites and worms in animals and is also used in the treatment of schizophrenia in humans. One of the proposed theories of schizophrenia is that it can be caused by a parasite infection (It has even been implicated in schizophrenia caused in some patients infected with the common parasite T. Gondii).

 

How piperine kills parasites isn’t exactly currently known. It has been theorized that it causes paralysis in parasites by increasing permeability of schistosome cell membranes that causes an influx of calcium ions.  This may cause the parasites to become dislodged from the site of action and killed by phagocytosis (engulfing by white blood cells) of the immune system.Piperine has also been possibly shown to decrease adenosine uptake by the parasites causing mitochondrial disruption which causes them to tire to death.


If this correlation is based in truth, then a simple bio-piperine extract in supplement form can be VERY useful in treating parasite infections. Piperine is a natural alkaloid of piperidine. Piperine has shown to eliminate parasite populations with less of the side effects of piperidine drugs. Piperine can increase absorption of supplements and medications and should be taken 2 hours before or after ingestion of medicines and supplements to decrease the chance of too much concentration of the medication or supplement in the blood.

 

Diatomaceous Earth / The Silver Bullet for ParasiteTreatment?

 

Diatomaceous earth is a naturally occurring rock made from the skeletons of fossilized algae. When food grade diatomaceous earth is grounded into a fine powder and ingested it can be used to kill of parasites in the human body. When the razor sharp edges of the diatoms come in contact with the parasites it punctures their cellular walls. It is recommended that the earth is mixed with water when taken so that the risk of inhalation is very low. The diatomaceous earth can be irritating to the lungs so try not to breathe in the dust.

 

I would suggest mixing ½ tsp in a glass of water twice a day (upon waking and before bed) for a week and slowly working up to one teaspoon twice a day. I wouldn’t use the diatomaceous earth drink more than three times a day. The diatomaceous earth should be taken for at least a month but no more than three months. Mix the diatomaceous earth with purified water.

I currently take 400 mg mag citrate at night, and have been upping my MCT oil as much as 5 tbsp. per day. This has definitely helped a little but I still only have about 2-3 bm's per week. Triphala to cleanse out the colon.  Vitamin C megadosage for the bowel laxative effect, look up vitamin c megadosage protocol on google. Use natural calm magnesium for more elemental magnesium that will make your bowls looser, 600 mg a day. Probiotics will help as well. Use organic hemp protein or organic chia seeds for extra "good" fiber. Sorbitol in prune juice would help as well. Two Cayenne pepper capsules in a glass of warm water and 1/4 tsp of sea salt will help as well. Make sure you are well hydrated the number one cause of constipation is dehydration.

Are you sure 600 mg is a safe dose for me. I only weigh 75 lbs. due to the lack of muscle mass in my legs. When I tried 600 mg for a few days last month it gave me diarrhea and made me a bit nauseous.

 

Given my body weight, how much vit C do you suggest? I currently take 1 g in the morning with my coffee, nothing at night though.

Also, I'm not sure if my calcium intake is high enough to allow for 600 mg of magnesium. Does a few cups of greens with dinner (spinach, broccoli, bok choy) give me over 1000 mg?

I've been eating raw carrots because of the lower glycemic rating. Do you think I should I switch to cooked for easier digestion?

Both are good depending on the application, raw during the day for the low gly, cooked at night or post workout. If there is carrot in your shit, cooked might be better or chew more, if you put cinnamon in the cooked there will be less effect from the gly index.
